US Size 10-11, 12-13, 1-2, and 3-4.

You will need an H crochet hook, 3.5 oz of aran weight yarn for the upper ( I used Patons Wool blend Aran shade 12), and a small amount of worsted in contrasting color for the sole (Red Heart Soft in Tan).
