I followed the pattern using the designated hook (G) (4 mm), but using a medium weight (worsted, 4) instead of the bulky as called for in the pattern. The size comes out to about an 8 for a woman using the largest size on the pattern (with 28 chains on foundation row). If needed, at top of toe, do two or three overcast stitches to close top of slipper. For larger sizes increase foundation chain in 4s - Size 7/8 has 28 chains on foundation row; Size 9/10 has 32 chains on foundation row; Size 11 has 36 chains on foundation row. Then adjust number of stitches along instep accordingly —i.e. add 4, 8, 12 to the required numbers before doing inc or dec in pattern. May have to add more rows along side if desired, but do before shaping of the last row 8 in the pattern. I did this for size 9/10 with 32 chains by repeating row 7 twice (for a total of 3) before doing final row 8 shaping.
