The circumference of the sweater is measured around the widest part of the body (stomach/chest).
1-3 months: 42 cm
3-6 months: 46 cm
6-9 months: 50 cm
9-12 months: 54 cm
12-18 months: body 58 cm
18-24 months: body 62 cm
1-2 years: 66 cm
2-3 years: 70 cm

E.g. if the indiviudal has a stomach measurement of 56 cm, I would knit the size 12-18 months (58 cm), so the individual has room to grow.
