There are 3 mitts sizes (0-3 months, 3-6 months, 6-12 months);

and 4 hat sizes in the PDF:

Size 1 (0-3 months) - 35 cm (13”1/2) head circumference
Size 2 (3-6 months) – 40 cm (15”1/2) head circumference
Size 3 (6-12 months) - 45 cm (17”1/2) head circumference
Size 4 (12-24 months) - 50 cm (19”1/2) head circumference
