This pattern is available for free.

Fits a standard or large UK take-away coffee cup and there are alternative instructions to make it fit round a mug.
You can also knit up this pattern using 2 strands of DK held together and it is great for stashbusting: you could stripe it too.
Requires six small buttons as fastening.
