I used a G hook for the foundation chain, and an F hook for the rest of the project.

I had a small pillow form. I just sort of eyed it up length and width, frequently draping or stretching the fabric until I had the right fit. I sewed the sides shut, turned it inside-out, and then I finished with a simple single crochet stitch along the bottom.
