Went with the second size. Rather than knit flat and sew up, knitted the front and back together in the round up to the armholes. Disregarded the length information in the pattern and instead went by measurements from an existing sweater. Modified the shoulder shaping to use short rows and then did a three-needle cast-off to join shoulder seams.
