I actually made this a couple of weeks ago but only got the time to actually write up the pattern in its proper form.

UPDATE: A typo was pointed out to me by sdorland88 (thank you btw :)). In the 2nd tier, in row 9, it should be hdc in the next 6 sps, not 4. An updated copy of the pattern will be uploaded to reflect the change.
